How do I become a new patient? Please complete a new patient packet and our staff will find an available appointment time that works best for you. How early do I need to schedule my appointment? Our providers’ schedules book up quickly. We always allow for walk-in appointments during the day, however we suggest scheduling in advance to insure you may be seen by your preferred provider. We recommend that preventative care visits be scheduled two months ahead– these include annual physicals and well-child checks. What do I need to bring with me to my appointment? You should bring the following items with you on the day you arrive: - Your health insurance card - Personal identification card (such as a driver’s license) - Medical records, x-ray films, or prior test results (if your physican has requested them) - Written letter of authorization from insurance (if you are covered by worker’s compensation) - Your medication bottles, including over-the-counter medicines, vitamins and herbs - Your insurance plan’s co-payment, co-insurance, or deductible for your visit Where can I smoke? The Family Doc premises are tobacco-free. That means we prohibit the use of all tobacco products on our grounds. Patients are not allowed to smoke. Assistance is available to help you stop smoking. Please ask your nurse for information.
Do I need my records transferred over before being seen as a new patient? As part of our new patient packet, we will request any medical records you have from previous primary care providers. We must have these records to treat any pre-existing conditions you may be experiencing.

What type of insurance do we accept? We file all insurance plans for all patients. We are preferred in-network providers with Medicare, Medicaid, Blue Cross Blue Shield of North Carolina, Cigna, MedCost, United Healthcare, Tricare and First Carolina Care Insurance Company. Please remember that in all cases of insurance, your policy is a contract between you and your insurance company. As a courtesy to our patients, we will make our best effort to facilitate that contract by any and all reasonable means. Do we see Pediatric patients? Yes! The Family Doc has always seen patients of all ages, as all of our providers are trained to see children. But, as of 2007, The Family Doc Pediatrics was opened for our younger patients. If you would like more information, please ask any member of our staff. Will arriving late shorten my appointment time with the Provider? We ask that patients arrive 15 minutes early for their appointment so we may have time for registration and any questions you may have about your insurance plan benefits. When a patient arrives late to their appointment, it can make the provider behind for the rest of their appointments for that day. So as a courtesy to all of our patients, the provider will make the decision if they can see late patients based on the availability of their schedule. If they are unable to see the late patient, the patient will need to reschedule at a later date. How do I contact my Medical Provider after hours? The Family Doc has someone on-call 24 hours a day for you and your family. By calling our main phone line after hours (919-776-3750), the answering service will link you to the on-call provider’s phone. Simply call the main number and follow the automated directions. If there is ever an emergency, please immediately call 911. Is there a charge for not showing up for my appointment? If you find that it is impossible for you to keep your appointment, we ask that you inform us of the cancellation at least 24 hours in advance. This courtesy allows us to be of service to other patients. If for some reason, we do not receive advance notice, there is a $50 charge for regular medical appointments and a $100 charge for Complete Physical Examination appointments.
